Policies

  • The process of establishing care at Recovery Medicine begins with making an appointment, completing the new patient paperwork, and making arrangements to pay for your appointment.

  • First-time patients will complete extensive paperwork, followed by a one-time 60-minute consultation. This comprehensive assessment will explore your history, current health status, needs and preferences.

  • Care is collaborative and a treatment plan will be developed.

  • Establishment of a doctor-patient relationship will be a mutual decision at the first visit.

  • I offer multiple types of follow-up visits. Although the time between appointments will depend on your individual needs and treatment plan, most patients will need to be seen at a minimum every three months.

  • Payment for all visits is due when you make an appointment.

  • Cancellations made less than 48 hours in advance will be charged a $25 rescheduling fee. Cancellations made less than 24 hours in advance will forfeit the entire visit amount. Cancellations made less than 24 hours in advance may be rescheduled with a $50 fee solely at the discretion of Recovery Medicine. We realize life happens and encourage you to communicate the circumstances of the cancellation or missed appointment so that we can continue to work together.

  • Recovery Medicine does not accept health insurance. You will receive a “superbill” at the conclusion of the visit which you may submit to your insurer for possible reimbursement.

  • Recovery Medicine does not support the use of intoxicants. If you are currently using intoxicants including alcohol, controlled substances, and/or illegal drugs we will create a safety together. We encourage rigorous honesty.

  • Not disclosing the use of intoxicants is detrimental to your healthcare and the patient-provider relationship. This can lead to termination of the relationship.

  • Disruptive behavior is unacceptable. All disruptive behavior will first be addressed clinically and if not resolved may result in termination of the patient-provider relationship.

Further information is provided on the new patient forms including your patient rights.
